血型（4）——母子ABO血型不合
在我上班查房的每一天，当我面对产妇时，我都会问她们一个问题：妳的血型是什么？
当她们回答是O型的话，我会心里一紧。随后就会问她丈夫的血型，如果她的丈夫也是O型的话，按照血型的遗传规律，她的孩子必然是O型（为什么？我会在下一篇的《血型和遗传》中探讨这个问题），我会松口气，这样就不会发生溶血；如果她的丈夫不是O型的话，我就得关注一下“ABO”血型不合的问题，因为她的孩子可能是A、B、O三种血型中的一种，当然O型母亲的孩子永远不会是AB型（也会在下篇中探讨为什么）
首先复习一下第一节课的内容：
A型血的人红血球表面有A型抗原；他们的血清含有对抗B型抗原的抗体
B型血的人跟A型血的人相反，他们红血球表面有B型抗原；血清中含有对抗A型抗原的抗体
AB型血的人的红血球表面同时有A型及B型抗原；他们的血清没有对抗A型或B型抗原的抗体
O型人的红细胞表面A或B型抗原都没有，可他们的血清对两种抗原都会产生抗体
问题是我们血清中的抗A或抗B抗体从何而来，我们每个人并没有接受过异型输血，而抗体不会在没有抗原的情况下产生，这就是我在第二节课提到的问题
这是一个困惑了人们很长时间的问题，但随着人类对抗原结构认识的不断深入，也就找到了答案：
在我们生活的自然界中有很多微生物、我们接种的部分疫苗、甚至部分食物（具体是哪些食物，书上也没细写，但欣燃回答是血豆腐，沾边，算是正确）具有和A型、B型抗原类似的物质。人类在微生物寄居下（例如我们肠道中就生活着大量的对身体有益的细菌）、进食或接种疫苗后，身体就会产生抗A和抗B的抗体。正是这些抗体导致异型间出现输血反应
好的，如果你很细心的话，你就应该提个问题： Rh阴性的人第一次输入Rh阳性人的血，为什么不会出现输血反应？Rh阴性的体内起初没有抗Rh阳性的抗体吗？
答案是：一个从未接触过Rh阳性血液的Rh阴性的人的体内，是不会含有抗Rh阳性的抗体，因为自然界中没有类似Rh阳性结构的物质，没有这些物质的刺激，Rh阴性的人是不会产生抗体的
好的，可以介绍一下ABO溶血病了：
1、一般只有O型的妈妈怀了A型或B型的孩子才有可能发生此病，而且第一胎就可发生。因为O型的妈妈血液里的抗A或抗B抗体会通过胎盘进入胎儿体内，继而有可能破坏孩子的红细胞
2、A型的妈妈怀了“B”型或“AB”型的孩子（或B型的妈妈怀了“A”型“AB”型的孩子），为什么就罕见发生溶血。就拿A型的妈妈举例吧，她的血液里可含有抗B的抗体啊，可她们的孩子绝大部分不受影响。这是因为这些对抗的抗体多是IgM，别忘了我上节课说的，IgM分子量大，透过不了胎盘，到达不了胎儿的体内，所以基本不会导致胎儿溶血。可O型妈妈血液里的抗体为什么就可能对孩子产生影响？原因是O型妈妈产生的对抗抗体是IgG，分子量小，可以透过胎盘
妳是O型血的话，也不要过于担心，因为如果妳要找了个O型的丈夫的话，妳的孩子必定O型，就不存在溶血问题；但妳的未来的丈夫不是O型的话，妳的孩子又不是O型的话（丈夫不是O型，孩子依然有O型的可能），虽然母子血型不合，但发生溶血的几率还是比较低的
因为A抗原和B抗原同时也存在于红细胞外的许多组织中，这样通过胎盘的抗A或抗B抗体仅有少量与红细胞结合，其余的都被胎儿身体里的其它组织和血浆中的可溶性A和B型物质中和，因此虽然母子ABO血型不合很常见，但发病仅占少数。只有当通过胎盘的抗A或抗B抗体浓度过高时，胎儿才会出现明显的溶血现象
而Rh血型不合就不同了，我们身体的Rh阳性因子基本只存在于红细胞表面，所以Rh阴性母亲生成的抗Rh阳性抗体进入胎儿血液后，基本只同Rh阳性的红细胞结合，所以破坏力相当的大
好了，今天的“母子ABO血型不合”简要介绍到此。在下一篇《血型与遗传》里，我将要解释父母是如何决定我们的血型的。还有，在临床上我们极其偶尔会遇到O型的父母生了一个不是O型的孩子，我们可以断定这个父亲不是孩子的生父，而我们又如何处理这个问题？我们的做法对不对？

关于（2）RH血型一篇中，老师留的思考题，我当时是这么想的：
a).我首先想到的一个可能是胎儿在母亲体内的时候，会不会在什么情况下接触到一些母亲血液里的抗原物质，想到这个，是因为老师说了，下节课要讲母子溶血什么的；
b).另一个可能就是注射的疫苗啊什么的，里面会不会有类似A抗原B抗原的物质呢？能想到这个，是因为在上一篇专讲抗原抗体的时候，我专门提问了关于卡介苗的事情，所以对疫苗什么的，特别敏感。
至于为什么，ABO溶血发生在第一胎，而RH溶血第一胎往往没事，我那时压根没思考这个问题，因为老师说了下一堂课要讲的么。
看了老师的（3）和（4），又产生一些问题，但做学生的也有忙的时候，我改天再来提问。

孩子的血液进入妈妈的身体，虽然血型不合，但进入的量极少，在流产或分娩时，胎儿学进入母体内的血液一般不超过1毫升，很快被母亲的血液稀释，对母亲不会造成影响，但这不到1毫升的血液，却足以使母亲产生足够的对抗孩子血型的抗体，而且这种生成抗体的能力被永远记忆

问题1）假设血型分别为A，B，AB，O血型的孩子们都注射了某种疫苗(暂且叫它疫苗甲吧），该疫苗既有类似于A抗原的物质，又有类似于B抗原的物质，那么A血型的孩子血清中会产生B抗体；B血型的孩子，会产生A抗体，AB血型的孩子血清中不会产生抗体，O血型的孩子，会产生A抗体和B抗体。是这样的么？
问题2)同是这四个孩子，他们在注射了疫苗甲之后一段时间，又接受了疫苗乙。如果疫苗乙当中也含有类似A抗原和B抗原的物质，会发生什么事？
我们小时候，都注射过好几种疫苗，但是大多数人都没事。所以，关于问题2，在老师回答以前，我自己先猜测一下。我们之所以没事，是不是因为疫苗中含有的类似A抗原B抗原的物质的量非常小，很快会被孩子的血液稀释掉，但是，却足以让孩子体内产生A抗体或B抗体，并且这种产生抗体的能力将一生被记忆？

1)举个例子，比如说A型人吧。无论他怎么接触含有A型结构的抗原物质，都不会产生对抗A型的抗体(你想，A型人自身红细胞上都有A型抗原，如果能刺激自身产生抗A的抗体，也用不着再靠外物的刺激产生了)。以此类推B,O,AB型吧
2)特殊情况下，机体的免疫功能出现严重问题时，可以出现对抗自身的抗体，称自身免疫性疾病，多为女性发病，例如系统性红斑狼疮。自身抗体破坏自身器官，预后多差
3）你的关于第2个问题的猜测应该是正确的。比如，A型人的体内有抗B抗体，但真的有极少量的B型血进入A型人体内，会有轻微的溶血反应，但立即被稀释，不会有严重后果。所以O型人的血清中尽管有抗A和抗B抗体，但O型的全血（包括红细胞和血清）进入其他血型人的体内，只要量不大，速度慢，也不会有严重问题，但这只在紧急情况下采用

每个O型的产妇体内的抗A和抗B浓度是不同的，当她体内的抗A和抗B浓度过高时，相应透过胎盘的抗体浓度也会增高，从而危及体内的非O型的胎儿，在怀孕其间我们可以多次通过抽她的血来动态监测这些抗体的变化从而预测发生溶血的可能
为什么每个产妇身上的抗A或抗B浓度不同，目前我还不清楚。也许被人研究出来了，但我不知道

关于两对半的核心抗体的问题，它不仅仅是IgM，也有IgG，意义如下：（我搜的）
